How to File a Complaint

Option 1 - Schedule an appointment with the individual involved or the Director of Health Services.

Option 2 - Download the Complaint/Grievance Form (pdf), complete it, and return it to Health Services. The Director will investigate the complaint and take action to resolve the problem insofar as possible.

You will be notified of the outcome of the investigation including any action taken. You have a right to appeal to the Dean of Students if you believe the outcome is unsatisfactory.
